here is the information we need:
1. Firm Name
2. Firm Address
3. Solicitors Name
4. Case Type - Workcover, Personal Injury,Negligence etc
5. How many complaints do you have? - Was there more than one thing that
was not done or completed?
6. Was it resolved? - Did you make a complaint to the law firm,
the ombudsman, Law Institute etc
